6 Week Program - Expression of Interest Form

(Australian and New Zealand residents only)

ONLINE GROUP MENTORING INFORMATION:

TECHNOLOGY REQUIRED: Access to a device that has both microphone and camera capabilities to access the Zoom platform.

COST: $321.00 AUD for a 6 session program

NOTE: The Dungeons & Dragons Campaign and Minecraft Survival program runs for two hours weekly over six weeks (12 hours total) at a cost of $642.00 AUD.

PROGRAM TIMES:

Afternoon: 5pm - 6pm AEDT

Evening: 6:30pm - 7:30pm AEDT

Payment

Will you be using NDIS funding for this program? We can issue Self Managed & Plan Managed (Not NDIA Managed) families with an invoice.

You must ensure sufficient funding is available to cover program costs, privately or through NDIA approval. If NDIS funding does not cover costs, you are liable for the balance. Non-payment, including from cancellations or no-shows, may result in recovery of funds through the NDIS and which could lead to your plan being managed by the NDIS Agency. We cannot provide advice on individual NDIS plans or funding.
